Community Case Management – CCM (ID)

Community Case Management (CCM) is a community-based program providing services to adults with intellectual disabilities. CCM staff assist in identifying medical, social, educational, and other needs of the client. CCM staff will address and help to facilitate access to these needed services. The case manager will also work with the individual and family to identify any wishes or desires and assist with achieving these goals. Case management involves plan of care development, coordination/advocacy, goal development and monitoring, and evaluation of services.
